Definitely looks like something toxic is involved.

What a horrible situation. I do hope you can get to the bottom of it soon.

Well that fish died, the remaining 2 are alive swimming well but wont eat, the filter in my tank is an internal aqua flow 200 and it had these little black bits in the middle of the filter i took them out a while ago, could that have anything to do with it?

The black stuff in the filter would have most likely have been carbon, that wouldn't have caused this. Do a big water change, as I still think it sounds like something toxic has got into the tank so you need to dilute what ever it is. Tapsafe wouldn't have caused it unless you massively overdosed but it would have to be massively.

How big are the worms?
Have you fed any live foods lately?
Can you take a piccy?

I think if it were my tank I'd be tempted to empty it
Give it a good scrub out with cooking salt, rinse and start again.
Wash the gravel until the water runs clear and is spotless. Wash the filters etc, throw away the white filter pads and start with new, give the sponges a good rinse.
Too much wrong here - I don't like the sound of that 'cloud' either, fungus perhaps from something rotting - bacteria.
You definately have a water issue...

The lfs told me to take all the water out etc. And clean it, but the problem is i have black gravel on rop of white so itll look a real mess. And ive used my gravel cleaner so its gone and cant take a pic. The worms were bloodworms that i fed to my fish. I was going to get a new sponge today then i realised im completey skint
